Important Information for 6th Grade Parents!

 All 6th grade students must turn in a new form 121 with the box checked compliant for 7th grade and above.

 We will hold next years school schedules until an updated form 121 is on file.

Doctor's offices run low on this vaccination so the earlier the better.

Bring in the new form 121 to the front office (not your student's homeroom teacher), fax it to 601 859-1302, or send it in to the front office with your student.
