HELPFUL INFORMATION: 

STUDENT WITH ASTHMA: If your child is being followed by a doctor for asthma, whether they need an inhaler at school or not, I am required by law to keep an ASTHMA ACTION PLAN on your student. You may request one from your doctor or come by the school to get one from me. It can be faxed to me at 601-853-2726. 

STUDENT WITH ALLERGIES: If your child requires prescription medication (EpiPen) for allergies, I will need an ALLERGY ACTION PLAN.  You may request this from your child's doctor or come by my office to get one from me.  It can be faxed to me at 601-853-2726. 

STUDENT WITH EPILEPSY: If you child has epilepsy or seizures of any kind, I will need a SEIZURE PLAN for your child. You can work with your doctor to get one - I do have some generic plans that I can provide, if needed.

 A Madison County Consent for Medication form is needed for ANY student needing to keep over the counter or prescription medication (other than for asthma or allergies) at school.  This includes but is not limited to, Tylenol, Benadryl, Cough Drops, anti-itch creams, Mucinex, ADD/ADHD, etc.  The MEDICATION FORM and STUDENT HEALTH FORM are under Resources and then Useful Formson the main Madison Avenue Elementary website.  

ALL medication MUST be brought to school by a parent/guardian.  ALL medication must be in original container, with current prescription label (if prescription). 

 If the prescription is a controlled substance (medication for ADD/ADHD) you will be required to count the medication with nurse or office staff upon delivery to verify quantity.  It does not have to be brought during the hours I am here.
