This form is required for every student enrolled in the MCSD. 

This form is required for any medication that needs to be administered during school hours. Per district policy, students may not carry medication on their person while at school. 

If your child has a life-threatening allergy (peanuts, shellfish, wasps, etc)., please have the student's doctor fill out this form. If your student needs to carry an EpiPen at school, the doctor will also need to fill out the Self-Carry form on page 2. 

If your child has asthma, please have the student's doctor fill out this form. If your student needs to carry an inhaler at school, the doctor will also need to fill out the Self Carry Form on page 2.

If your child has a history of seizures, please have the student's doctor fill out this form.
