You must provide two different current utility bills (electric, water, gas, land phone) that clearly show your name, street address, service address and date. No cell phone bills will be accepted to verify residency. Any document with a post office box as an address will not be accepted, unless the service address is listed.
IF YOU LEASE YOUR HOME:
- You must submit a current lease agreement signed by the homeowner. (all occupants must be listed on the lease)
- You will also need to provide two different current utility bills (electric, water, gas, land phone) that clearly show the street address and name shown on the lease.
IF YOU HAVE MOVED (to a new house in our zone):
- You must bring a copy of your new deed or lease.( all occupants must be listed on the lease)
- You must also complete a new affidavit of legal residence in the school office.

Immunizations - Current 6th graders MUST have a Form 121 ON FILE to completely register for the new school year!
The state requires that this form have records of the new T-dap vaccine.
