Click HERE to complete the PTO Membership form.  While completing the form, you will notice that you have three options from which to choose how you turn in your information to the PTO.  Please mark your preferred option and make sure that you send your information to mchspto@gmail.com by clicking on the "Submit by Email" button found at the top of the Form.  If your email account does not automatically send the file, please save it to your computer and send it as an attachment.   The file name will default to MCHS_PTO_Membership_2012-13.xml.  If emailing the information is not your preferred option, feel free to either mail or hand deliver your completed form and/or payment to the following address:
